Highmark Health Services and St. Luke's Begin Patient-Centered
Medical Home to Help Members in the Lehigh Valley

Camp Hill, PA (8/28/2013) - Highmark Health Services and St. Luke's University Health Network, a six hospital system, will begin work on a patient-centered medical home (PCMH) program.

Through a PCMH, physicians take greater accountability in coordinating the care for patients. This accountability means assisting patients and their families with treatment and treatment options. It also means better patient education and better use of tools such as electronic health records.

“St. Luke's has provided high quality care for people of the Lehigh Valley for many years and we are pleased to be collaborating with them on this effort,” said Matt Vogel, vice president for provider contracting and relations for Highmark Health Services. “Based on our extensive pilot, we will continue to focus on high quality while controlling costs.”

Highmark Health Services completed a pilot PCMH program that included more than 1,000 doctors and touched more than 170,000 members. Some of the key findings from the pilot:

  • Nearly 2 percent reduction in costs
  • Inpatient acute admissions dropped on average 9 percent
  • 30-day readmission rate dropped on average 13 percent

As part of the PCMH, more information is exchanged between clinicians with the goal of more coordinated care. It is also includes shifting people away from emergency rooms and getting the chronically ill into more regular interactions with their doctor.

This PCMH will have ongoing evaluation as to the effectiveness of the program.

Highmark Health Services is focused more on paying for value rather than just paying for the volume of services that has been performed. Highmark Health Services hopes to have about 75 percent of members in the pay-for-value arrangement in the next several years.
