Prescription Refills

For prescription refills, please call our office between 9 am to 4 pm Monday to Friday. Keep in mind that your physician may not always be available to refill your prescription. Please allow 24 hours for a request to be completed.

It is important that you speak slowly and leave the following information:

  • Spell the patient's name
  • Date of birth
  • Home telephone (work and cell numbers if applicable)
  • Pharmacy location and phone number
  • Name of medication, quantity and dosage
  • Prescription number on the bottle
